Surgery without sufficient indications: an update from Russia

This review is an update and continuation of preceding papers on invasive procedures applied in Russia with questionable clinical indications. Certain methods have been applied according to instructions by healthcare authorities and publication of leading specialists, facilitated by lacking professional autonomy. Examples are discussed here and in preceding papers: the overuse of Halsted and Patey mastectomy, preventive electrocoagulation of cervical ectropions without cytotological examination, gastrectomy for peptic ulcers, thoracic and abdominal surgery for bronchial asthma and diabetes mellitus, overuse of surgery in tuberculosis and other pulmonary conditions, overuse of bronchoscopy e.g. in conscripts with supposed pneumonia. Among mechanisms contributing to the persistence of suboptimal methods has been the autocratic or military managerial style discouraging criticism and polemics. Other attributes of this style are the paternalistic approach to patients and insufficient adherence to the principle of informed consent. Some invasive procedures with questionable indications were advocated by first generation military surgeons. Personnel training could have been one of the motives. Note that military and medical ethics are not the same. The comparatively short life expectancy in Russia is a strategic advantage as it necessitates less healthcare investments and tensions. Actually, Russia needs foreign help in the matter of healthcare. In view of the current international tensions, the cooperation has been partly interrupted. Obstacles to the import of medical products have adverse consequences. Under these circ*mstances, the purpose of this article was to remind that, performing a surgical or another invasive procedure, the risk-to-benefit ratio must be kept as low as possible. Insufficient coordination of medical studies and partial isolation from the international scientific community can result in parallelism in research, unnecessary experimentation, and application of invasive methods without sufficient indications.

Surgical procedures with questionable indications used in Russia

This review summarizes available data on invasive procedures applied with questionable indications in Russia in the recent past: mastectomy with the removal of muscles, gastrectomy for peptic ulcers, splenorenal anastomosis for diabetes mellitus, thoracic surgery for bronchial asthma, overuse of surgery in tuberculosis, excessive and compulsory treatments of alcohol-dependent patients, etc. Among factors contributing to the use of invasive procedures with unproven efficiency have been the partial isolation from international scientific community, insufficient consideration of the principles of professional autonomy, informed consent and scientific polemics, training of medical personnel. It is known that invasive procedures can exert a placebo effect, which might have contributed to reported efficiency of some methods discussed here. However, by definition, placebo must be free of risks.

Some Aspects of the Surgical and Endoscopic Treatment of Tuberculosis in Russia

After the introduction of efficient drug therapy, the surgical treatment of tuberculosis (Tb) has been partly abandoned in many parts of the world. The priority of Russia in the field of Tb surgery was pointed out. An overview of literature on the surgical treatment and bronchoscopy in Tb is provided here. Despite the lack of reliable clinical trial data on efficacy and safety of adjunctive surgical therapy, some countries of the former Soviet Union continue to perform numerous lung resections. Tuberculoma has been one of the most frequent indications for lung surgery in Tb patients, which is generally at variance with the international practice. Besides, the use of bronchoscopy in Tb is briefly overviewed. In conclusion, some papers containing questionable recommendations have remained without due commentaries, so that a persistence of suboptimal practices or reversion to them is not excluded.

Magnesium Supplementation with Special Reference to Alcohol-Related Conditions: Experience from Russia

Intravenous infusions of magnesium-containing solutions were routinely applied in Russia as treatment of alcohol withdrawal syndrome and other alcohol-related conditions. However, this indiscriminate approach is unfounded and may cause more harm than benefit. Magnesium deficiency is associated with certain circulatory, neuropsychiatric and metabolic conditions. The frequency of hypomagnesemia is enhanced in cases of chronic alcoholism. However, moderate deficiency does not necessarily require parenteral magnesium supplementation. Iatrogenic hypermagnesemia is associated with adverse effects. Endovascular manipulations in conditions of suboptimal procedural quality assurance can cause transmission of viral hepatitis. To decide if an intravenous magnesium supplementation is indicated, it should first be determined whether there is a deficiency and, if so, whether it can be compensated by diet or by oral intake of magnesium-containing drugs.
